2025 Community Information Form

Each community is required to complete the 2025 Community Team Information Form in order to play Hot Stove Baseball or Softball in 2025. This must be completed no later than December 1, 2024.

Only 1 form per community will be permitted and must be completed by a community official.

INDIVIDUAL TEAMS MUST NOT COMPLETE THIS FORM!
